PTSD develops after experiencing or witnessing a traumatic event. The nervous system becomes stuck in a state of threat — replaying the trauma through flashbacks and nightmares, and keeping the person in a state of hypervigilance that is exhausting and debilitating. Specialist therapy can resolve PTSD completely.

What Is PTSD?

PTSD can develop after any event involving actual or threatened death, serious injury, or sexual violence — either experienced directly, witnessed, or learned about in a close person. Not everyone who experiences trauma develops PTSD — resilience factors and social support play a significant role. In Pakistani contexts, common causes include road traffic accidents, domestic violence, sexual violence (which carries enormous stigma), witnessing violence, and significant medical events. Many people do not seek help because they do not know that what they are experiencing has a name and is treatable.

Causes & Risk Factors

PTSD is caused by trauma — but not everyone who experiences trauma develops it. Risk factors include: severity and duration of the trauma, previous trauma history, lack of social support, female sex, dissociation during the event, and prior mental health conditions.

Re-experiencing: flashbacks, nightmares, intrusive memories

Avoidance: of people, places, thoughts, or feelings associated with the trauma

Treatment at ASPI

PTSD has two highly effective trauma-focused treatments: EMDR (Eye Movement Desensitisation and Reprocessing) and Trauma-Focused CBT. Both are available at ASPI. These therapies do not require the patient to talk extensively about the trauma — they work by processing how the trauma is stored in memory.

EMDR (Eye Movement Desensitisation and Reprocessing)EMDR تھراپی

EMDR is an evidence-based therapy specifically developed for PTSD. It uses bilateral stimulation (eye movements or tapping) while the patient briefly recalls traumatic memories — helping the brain reprocess and "file" the memories correctly.

Trauma-Focused CBT (TF-CBT)صدمہ مرکوز CBT

TF-CBT is the other gold-standard treatment for PTSD. It includes psychoeducation about trauma, relaxation skills, and gradual processing of the traumatic memory in a safe, structured way.

Medicationدوائیں

SSRIs (particularly sertraline and paroxetine) are approved for PTSD and can reduce symptom severity, especially when therapy alone is insufficient or when depression co-occurs.

Myths About PTSD

"You just need to forget about it and move on."
Myth

Telling someone with PTSD to "forget about it" is not helpful advice — it is physiologically impossible. The trauma is stored differently in the brain and cannot simply be dismissed. Therapy works by changing how it is stored, not by suppressing it.
